PLEASE be advised: There is a 48 hour cancellation/ No Show Policy. Please read all patient handouts.

Come prepared to Consultation/Coaching Session with question, goals, and any other pertinent information. Be ready to
be clear and focused on the appointment as our time is very valuable.
