What to Expect?
At your first visit, Dr. Sly will do a thorough intake, listening and learning about your
individual health concerns and identifying your specific needs. The first two visits are best scheduled within a week of each
other so that Dr. Sly has time to review your case, research drug-nutrient-herb interactions and gather more information before
coming up with a plan. A plan will be developed which may include dietary and lifestyle recommendations, botanicals, homeopathy,
nutritional supplements, hydrotherapy or other natural therapies. The first session will last an hour and follow up sessions
last 30 minutes. It is imperative that you have downloaded the patient handouts (located on the patient handouts page),
filled in ALL information requested, sign, and send to:

Patient Handouts MUST be received by Dr. Sly prior to first session.
Payment
Cash or Credit accepted.
Payment MUST be received prior to time of appointment.
Cancellation Policy
In consideration of other patients and Dr. Sly's time, kindly allow 24 hours notice
to cancel appointments or reschedule your appointment. Late (Greater than 15 minutes)or missed appointments will be billed
$50.
If Dr. Sly needs to cancel your appointment, she will promptly refund you $50
